The client: a woman 91 years 160 cm 43 kg
A 91-year-old senior suffers from moderate dementia, characterized by forgetfulness and occasional hallucinations. She is not aggressive and does not wander, but she tends to fabricate false events. She drinks and eats little, suffers from incontinence (wears incontinence briefs), and has an increased risk of falls. She moves with a walker and requires assistance with hygiene, dressing, getting up, toileting needs, and medication adherence. Home care responsibilities also include household maintenance, cooking, and shopping (6 km by car). Companionship is important, including simple conversations, TV, games, and walks. At night, assistance is needed once for toileting. Relatives live next door and have a small dog that visits the senior.
