The A1 form is a document that serves to confirm that a person working in one European Union (EU) country is insured under the social security system of their home country. For caregivers working abroad, the A1 form is important as it confirms that they pay social and health insurance in Slovakia and do not need to do so in the country where they are currently working. To apply for the A1 form, you must have an active trade license in Slovakia for 2 months. After submitting the application, you can travel abroad as a caregiver, but you must provide us with a copy of the application for the issuance of the A1 form.
Procedure for issuing the A1 form:
1. The self-employed person (SZČO) fills out the application: The application is submitted at the relevant branch of the Social Insurance Agency, which records, checks, and if incomplete, supplements it (in cooperation with the SZČO). Attachments to the application: • Photocopy of the Service Mediation Agreement and Rahmenvertrag über Betreuungseinsätze im Bereich einer häuslichen 24-Stunden-Betreuung (both contracts will be provided by us) • Photocopies of 2 invoices by which the SZČO declares the performance of activities in Slovakia
2. If all conditions are met by the SZČO, the relevant branch of the Social Insurance Agency issues the portable document A1 no later than 45 days from the date of receipt of the application and sends it to the SZČO. The form is valid for the period for which it is issued. The headquarters of the Social Insurance Agency issues the portable document A1 for a maximum period of 24 months from the date of receipt of the application. After 24 months, it is possible to apply for its extension.
3. Abroad, it is necessary for the SZČO to have the portable document A1 with them, in case its presentation is required by a foreign entity, control and inspection authorities of the given state, if the SZČO suffers a work-related injury, and similar situations.
4. The Portable Document A1 does not need to be translated into another language because a uniform format is used by all EU, EEA countries, Switzerland, and the United Kingdom of Great Britain and Northern Ireland.
5. Any changes that occur during the period of assignment must be reported in writing by the self-employed person to the relevant branch of the Social Insurance Agency within ten days.
